IQ TESTING SCHEDULING
Book an IQ appointment via our online scheduling system using the links below!
Contact our Client Care Coordinator if you are having difficulty finding an appointment for your child.
Fees and testing days vary depending on the clinician.
Find the name of the test that your child needs, and then choose from among the clinicians and locations listed below that specific test. Please only book with one clinician at a time.
*NOTE: Please use your child's name when registering (along with your email and phone number).
When booking, the system will prompt you to enter a credit card and pay a $10 deposit to hold your testing appointment. Those clients who wish to pay via cash, check or Zelle should send the remaining balance in advance or bring payment with them on the day of the appointment.
Checks should be made out to Mind Matters, and mailed to 721 Monterey Blvd, San Francisco, CA, 94127
Zelle payments should be sent to rebecca@sfmindmatters.com
**NOTE: If we have not received payment on the day of testing, we will charge the card on file.
