Fee Adjustments for Mind Matters Evaluations

Mind Matters offers a limited number of fee adjustments for evaluations and screenings, based on family household size & total income.

Eligibility

Find the column with your family size.

If your total combined household taxable income falls at or below the amount listed in Level 4 for your family size, then your family qualifies for a fee adjustment.

Fee adjustments occur on a percentage basis:

At or below Level 4: 10% adjustment

At or below Level 3: 20% adjustment

At or below Level 2: 30% adjustment

Supporting Documentation

To verify eligibility for a fee adjustment, please provide one of the following:

First page of most recent tax return (preferred)

Current Medi-Cal card

GA Statement of Benefits

SSI determination letter

Availability

Note that only some of our clinicians can offer fee adjustments, thus spots are limited.

If a fee adjusted spot is not immediately available, please ask to be added to our waiting list for a future spot.
